How to fill out Texas VTR-34-S

The certified copy application for a vehicle already carrying a salvage or nonrepairable title. It is a separate form from the ordinary VTR-34, and it goes to a different address.

Two documents, and they are not the same thing. Take the blank VTR-34-S from the txdmv.gov copy so you get the revision that is current today, then print our VTR-34-S guide and keep it beside you while you fill theirs in.

Written against VTR-34-S (Rev 08/16), issued under Texas Transportation Code 501.134. Check the department record for an old lien before you post this. A lien somebody stopped thinking about in 2014 is still on the record until somebody files to take it off.

It is a short form, and the length is misleading - the weight of it sits in the instructions above the fields, which run the mailing addresses, the original-documents rule and the warning about a lien that is paid but never removed from the record.

VIN, year, make, body style and model of the vehicle already carrying a nonrepairable or salvage title.
Watch out: This is the salvage counterpart of the ordinary certified copy application. Using the wrong one of the two sends the paperwork to the wrong address as well as the wrong desk.

Title or document number
The number off the missing title, and the form adds if unknown, leave blank.
Watch out: Worth ten minutes of looking before you give up on it - an old registration receipt or a lender's payoff letter often carries it, and it shortens the search at the department's end.

To whom the certified copy will be mailed: name, address, and an optional email and phone number.
Watch out: Posted to the Vehicle Titles and Registration Division in Austin - a PO Box for ordinary mail and a street address for overnight and express. Not to a county office and not to a Regional Service Center.

Certification and signature
You certify you are the recorded owner, lienholder or verified agent and that the original salvage or nonrepairable title has been lost, destroyed or mutilated, then sign, print and date.
Watch out: Check the department record for an old lien first. The form's own warning is that even a lien paid in full remains on the record until an application for corrected title is filed, and until then you have to supply an original release.

What gets this one rejected

•Sending it where the ordinary VTR-34 goes. Two similar forms, two different addresses.
•Leaving a paid-off lien on the record. The form warns that even a lien paid in full stays on the department record until an application for corrected title removes it - and if it has not been removed, an original release of lien has to come with this.
•Photocopied court documents.
•Cash in the envelope.
