The stand-alone version of the certification that appears on the title, the bill of sale and the application: one page, one reading, three code boxes, two signatures. The division accepts the disclosure in any of those four homes, so this sheet exists for the sales where none of the others worked out - an assignment with no room, an application not yet drafted, or a transfer where the odometer statement is the only piece still missing.
Written against MVD-10187 (Rev 07/12), issued under NMSA 1978 ยง66-3-101(A); 49 CFR 580.5. The heading paragraph is blunter than most: failure to complete an odometer disclosure statement, or providing false information, may result in fines and/or imprisonment. It is the shortest form in the New Mexico set and the one with the longest reach.
Two documents, and they are not the same thing. Take the blank MVD-10187 from the realfile.tax.newmexico.gov copy so you get the revision that is current today, then print our MVD-10187 guide and keep it beside you while you fill theirs in.

Watch out: Copy the VIN from the certificate, and check it against the windshield plate once. This sheet travels with the title packet, and a VIN mismatch unstitches the pair.

Watch out: A digital cluster that will not light up gets no invented number: record what can be read, or leave the figure blank with the honest code, and expect questions - that is the system working.
AM for a true reading, EL for an odometer past its mechanical limit, NM when the reading is not the mileage.
Watch out: A five-digit odometer on a long-lived car is the EL case the manual itself walks through: 24357 showing on a car that has plainly travelled further is disclosed as 24357 with EL, not as a guess at the true total.

Watch out: Print, don't sign, in these boxes - the signatures have their own lines below, and a signature where a printed name belongs is the kind of tidy-up this document cannot absorb.

Watch out: The date here should match the date of sale on the assignment. These sheets are read side by side, and dates are the easiest discrepancy to avoid.

Watch out: Unsigned by the buyer, this is not a disclosure - and the missing signature is discovered at the counter, on the buyer's deadline, with your phone ringing.
