The one filing in a Kansas sale that exists purely for the seller, and the Division has moved it entirely online: there is no TR-216 PDF on the forms page, and the FAQ's own link for the words Seller's Notification of Sale, Form TR-216 goes to the iKan payment portal. Ten dollars files a statement that you assigned the title on a named date to a named buyer. The statute pays that back three ways - the filing is prima facie evidence of the assignment, it creates a rebuttable presumption, and if the buyer never applies for registration you shall not be held liable for damages resulting from the operation of the vehicle.
Written against TR-216 (not published online), issued under K.S.A. 8-135(c)(12). Read the certification twice before ticking it. You are certifying that the assigned certificate was delivered to that buyer on that date, and the portal's own capitals warn that false certification can result in criminal prosecution - so a seller who files optimistically, with the title still on the kitchen table waiting for a co-owner's signature, has sworn to a delivery that has not happened.
There is no download link for the TR-216 here because there is not one anywhere - the Division of Vehicles does not publish it. The card below explains where it does come from.
There is nothing to show filled in because there is nothing to fill in on paper: the Division publishes no TR-216 PDF, and its own FAQ links the form's name straight to the iKan portal at pay.payitgov.com, where the filing is typed into a web form - VIN, year, make, model, date of sale, both parties' names and addresses, the sworn certification of delivery, and a ten-dollar payment.
