The third member of Idaho's duplicate family, and the one for a title that is missing and wrong at the same time. Its own summary is the cleanest: use this form when a certificate of title has been lost or damaged and an owner or lien is being added, removed, or changed. That is what separates it from ITD 3367, which reproduces a title exactly, and from ITD 3369, which sells one on. The important limit is in the instructions rather than on the face: at least one name on the original title must remain on the title - so this is a form for adding a spouse, dropping a co-owner or moving a lien, not for handing the vehicle to a stranger.

The instructions refer to the release of interest as ITD 3377. The department's own forms index, and the form itself, number it 3777 - ask a lienholder for a Release and Satisfaction Of Interest by name rather than by that number, and check the footer of whatever comes back.
Two documents, and they are not the same thing. Take the blank 3371 from the itd.idaho.gov copy so you get the revision that is current today, then print our 3371 guide and keep it beside you while you fill theirs in.

Both lien sections are washed grey along with the mail-to-a-different-address block, because there is no lender anywhere in this transaction - and Section 5 only works where a lienholder in Section 3 has signed Section 7 anyway. The notary block stays empty: it is a printed rule with no field behind it.

Section 3 is the lien that should appear on the new title; Section 4 is where an old lender releases one that has been paid.
Watch out: Do not use Section 3 to record a lien you have finished paying. That posts your new title to the lender you just cleared.

Only usable where a lienholder shown in Section 3 has signed Section 7 and named the recipient.
Section 6 - Fees Paid, with a credit card block
$14.00 for the Idaho title, or $40.00 in total with the $26.00 RUSH fee added. Cash cannot be posted.
Section 7 - Owner's or Lienholder of Record's Signature with Notary
The affidavit half: the original title is presumed lost unless the Illegible or Mutilated box is ticked, in which case the old title is attached.

Subscribed and sworn before me, with a seal or stamp area, or an assessor's or deputy assessor's witness in its place.
What gets this one rejected
•Removing every original owner. At least one name on the original title must remain on the title - dropping all of them is a transfer, and a transfer goes on ITD 3369 or on the certificate itself.
•Removing a co-owner joined by AND without their release. The instructions require a release of interest from the party being removed where the names were connected by AND.
•An un-notarised signature. The banner is the same as ITD 3367's: this form will NOT be processed unless the applicant's signature has been NOTARIZED.
•Listing a lien in Section 3 that has already been satisfied. The instruction says not to, and a lien listed there redirects the new title to the lender.
•Sections 1, 2 or 7 left incomplete. The instructions name all three as required to be filled out completely and correctly.
•A faxed copy. Not acceptable, in the form's own words.
Rules that decide whether it is accepted
•A changed name needs a bridge, not a correction. The instructions: if any name has changed since the original title was issued, for example by marriage, include a signed One and the Same Statement, and ITD 3125 may be used for it.
•The lienholder's signature in Section 7 is not a release. Same warning as on ITD 3367 - a signature from the lienholder in Section 7 does not release their lien.
•It has a postal address of its own. Unlike ITD 3367, this form prints where to send it: the Ada County Processing Center, PO Box 140019, Garden City ID 83714, or by courier to 9115 Chinden Blvd, Ste 103.
