The one North Dakota form a seller can genuinely need and cannot download. It is a federally secure document, and the department does not publish it: the Registration Manual names it and then tells you where it lives, in brackets, in the middle of a sentence - contact MVD for form. Three other forms send you to it. The loose Part 1 and the dealer's Part 2 both bar themselves from odometer-required vehicles and name this as the substitute; the statement of error requires a separate conforming secure odometer disclosure alongside it; and the odometer correction lists it as document two of five when the error happened during an ownership change.
Written against SFN 18773 (not published online), issued under North Dakota Department of Transportation, Motor Vehicle Division; the federal odometer disclosure the Registration Manual applies under NDCC 39-05-05(1)(g). This is the gap to plan around if your car is model year 2011 or newer and its certificate's own Part 1 has been spoiled. Every published alternative excludes odometer-required vehicles by name, which leaves one route, and that route runs through a telephone call to Bismarck. Sort it before you advertise the car rather than on the day somebody arrives with cash.

There is no published PDF to fill in. This is a federally secure form that the North Dakota Department of Transportation does not put in its forms index or anywhere else on dot.nd.gov, and the Registration Manual's own instruction for obtaining it is to contact the Motor Vehicle Division - so showing it completed would mean showing something we had not read.
