The application that converts a Maine certificate of title into a certificate of salvage, and the second of the three Maine forms that exists only on paper. The Bureau says where to get it in its own words: the application can be obtained by visiting any of our motor vehicle branch or town office locations, or by contacting the Title Unit in Augusta. A private owner meets it in one specific situation - deciding that a damaged vehicle is worth more in pieces than on the road, which is a declaration the owner is entitled to make without waiting for an insurer.
Written against MVT-102 (10/18, read from the Bureau's own blank specimen), issued under 29-A M.R.S. section 654. Declaring your own car salvage is not reversible on a whim. It puts a brand on the record that follows the vehicle through every subsequent sale, and the road back to an unbranded certificate does not exist - the best available outcome afterwards is a rebuilt or repaired code, which is itself a brand.

There is nothing to render. Like the title application and the transfer sheet, this is a multi-part carbon form the Bureau issues over a counter and publishes no PDF of, so there is no AcroForm behind it and no honest way to show one completed.
