DFA's general proxy instrument, for the seller who cannot be at the counter. Its own opening paragraph defines its two uses: a person applying for a Motor Vehicle transaction on behalf of a company or individual, or a person accepting delivery of a title on behalf of the titled owner - and it must be submitted to the State by the person exercising power of attorney. It is valid until revoked, which the form states in bold on its own line, and it dies instantly on correction: this document is void if any information is blank or any information entered hereon is erased or altered by any means.
Written against Vehicle POA (no revision printed), issued under Ark. Code Ann. Title 27, Chapter 14. DFA publishes this form as a scan, and its warning about false statements is the sheet's only enforcement language - which cuts both ways. Grant it only to someone you would hand the signed title itself, because between appointment and revocation they can sign your name to all applicable documentation relative to any Motor Vehicle transactions for the vehicle described.
Two documents, and they are not the same thing. Take the blank Vehicle POA from the dfa.arkansas.gov copy so you get the revision that is current today.
There is no completed sample of this form on this page because DFA publishes it only as a scan with no fillable fields and no text layer, and its writing lines are printed as faint dotted rules the box-reading fallback mostly cannot resolve: it recovers the two company checkboxes and three of the appointment rules, but not the granter's own name blank, not one cell of the vehicle information table, and none of the signature, address, telephone or email rules a sample would actually need. Rather than place values on lines the document itself cannot locate, the field list above names every blank in the order the sheet asks for them.
