Replaces a Nevada title that has been lost, stolen or mutilated. Only for a vehicle last titled in Nevada - a title from another state has to be replaced by that state.
Written against VP 012 (4/2026), issued under NRS 482.438 and NRS 490.082. This revision is dated April 2026 and replaced an earlier one. A duplicate title application on an out-of-date form is the kind of thing that costs you three weeks in the post.

| Requested by | Only filled in when the duplicate is to be mailed to a different address or person - and then an original notarized authorization letter is required unless the recipient is a licensed Nevada dealer or the legal registered owner. |
| Signature | The application must be notarized, or witnessed by an authorized Nevada DMV representative. It cannot simply be signed at the kitchen table and posted. |
